MAXILLA
Limiting Structures
Labial frenum
Contains no muscle fibres
Accommodated by a groove in labial flange of maxillary denture
Labial vestibule
Contains no muscle fibres
But is indirectly effected by Orbicularis Oris muscle
Buccal frenum
Band of tissue that overlies the Levator anguli Oris/ Caninus muscle
Buccal vestibule
Bounded Laterally : cheek Medially : RAR
Area of the denture which will fill this space is known as Buccal Flange
Hamular notch
Posterior palatal seal(vibrating line)
Fovea palatina
Supporting Structures
Primary stress-bearing areas
Hard palate
Mucosa is keratinized throughout
Posterolaterally, submucosa contains glandular tissue
Horizontal area lateral to midline provides primary support
Postero-lateral slopes of residual alveolar ridge (RAR)
Denture bearing area of the ridge is covered by : KERATINISED MUCOSA
Secondary stress-bearing areas
Rugae
Maxillary tuberosity
Relief areas
Incisive papilla
It overlies the incisive foramen
nasopalatine nerves & vessels arise
Relief over the papilla should be provided to avoid pressure on nerve & blood supply
Cuspid eminence
Mid-palatine raphe
It is relieved to - prevent pressure on palatal region
Mid palatine suture area is covered by thin submucosa & mucosa layer is in contact with the underlying bone.
The soft tissue is non resilient & is relieved to avoid trauma from denture base
OTHER KEY POINTS
Massetric Notch
In distobuccal corners of mandibular denture
Action of Masseter on buccinator
Limits of lower denture
Anterior region
Origin of Genioglossus muscle & sublingual gland
Molar region
Mylohyoid muscle
Distal most part
Palatoglossus & superior constrictor
Average denture bearing area
Maxilla
24 cm2
Mandible
12.25cm2 / 14cm2
MANDIBLE
Limiting structures
Labial frenum
Fibrous band, Active in nature
Influenced by Orbicularis Oris & Incisivus muscles
Labial vestibule
Buccal frenum
Buccal vestibule
Lingual frenum
Alveololingual sulcus
Extends from lingual frenum to retromylohyoid curtain
Anterior region
extends from lingual frenum to premylohyoid fossa
Middle region
extends from premylohyoid fossa to distal end of mylohyoid ride
Posterior region
Retro-mylohyoid fossa is present here
Denture flange in this region should turn laterally upwards towards ramus to fill up the fossa
complete the typical S-form of lingual flange of lower denture
Retromolar pads
Relatively stable landmark
Should be covered by lower denture for perfect border seal
Boundaries
Posteriorly - Temporalis tendon
Laterally - buccinator
Medially - pterygomandibular raphe & superior constrictor muscle
Mylohyoid ridge
Irregular bony crest on lingual surface of mandible
The ridge is near Inferior border of mandible in the incisor region & terminates near 3rd molar area
Border of lingual flange may extend below mylohyoid line if it slopes towards the tongue
External oblique ridge
Supporting structures
Buccal shelf area
Boundaries
Medially - crest of the ridge
Distally - retromolar pad
Laterally - external oblique ridge
Residual alveolar ridge
Relief areas
Crest of RAR
Mental foramen
Lies on external surface
Between 1st & 2nd premolar area
In severe resorption of the ridge - denture should be relieved over the foramenn
To prevent pressure on the mental nerves and vessels
Genial tubercles
Paired bony tubercles on Anteriorly lingual side of body of mandible
Due to resorption, may become very prominent making denture usage difficult
Torus mandibularis
