- Elderly
- Have you ever had rheumatic fever?
- Have you been told you have a cardiac murmur before?
- Do you ever get chest pain or SOB with exercise?
- Known bicuspid valve?

HOCM
- Do you have a known heart disease in your family?
- Family history of sudden death?
- Do you ever get chest pain, feel SOB or like you're about to faint?

Fast Recovery

Simple/ Vasovagal
- Onset in teens or 20s
- Response to emotional distress eg sight of blood, venesection, pain
- Prodrome of nausea and clamminess
- Falls to the ground, unconscious <2min
- Jerking movements and incontinence can occur
- Recovery within seconds, esp if lie down
- Persistent jerking and post-episode confusion are UNCOMMON and suggestive of a fit rather than a faint.

Orthostatic Hypotension
- Onset when getting up quickly
- Brief duration, more common when fasted or dehydrated
- Known low systolic, or on antihypertensives or diuretics
- If reflexes are absent eg diabetic or alcohol abuse

Cardiac Arrhythmia
- Non-exertional
- Family history of sudden death
- History of cardiac disease
- History of palpitations
- On anti-arrhythmic medication
- NO WARNING
- MI or AS- has anyone said you

Slow Recovery

Seizure
- Prodrome- aura
- Tongue bitten
- Incontinence of urine and feaces
- Jerking movements
- Cannot remember the incident- post-episode confusion
- Sleepy/ exhausted afterwards
- Muscle pain afterwards
- Lowered seizure threshold eg on tricyclic anti-depressants
- On/ withdrawing from drugs/ alcohol
- Period of emotional stress/ sleep deprivation
- Previous incident of head injury or stroke -> post traumatic ep
- Meningitis- neck stiffness? vomiting? fever?
- Hypoglycemic- Diabetic? Medications? Well managed?
- History of progressive headache, worse with waking or coughing/ sneezing, or any abnormal weakness/ sensations developing

Pulmonary Embolism
- Sudden onset of retrosternal pain, shortness of breath and then collapse
- Prev history of DVTs
- OCP
- Known malignancy
- Recent surgery or air travel
- Coughing up any blood

Meniere's/ Vertigo
- No LOC- sudden severe dizziness can cause 'drop attacks'
- Room spinning around them
- Worse/ Occurs when they turn their head too quickly
- Tinnitis? Deafness? -> meniere's.
- Do you find your a bit nervous walking down stairs or crossing the road?
